Medication Assisted Treatment Intake Form

Medication-assisted treatment (MAT) is available to support those who struggle to maintain sobriety through counseling and community support alone. While FDA-approved medications may be used as an aid to recovery, they are not a substitute for the work of recovery. Learning new ways of thinking, changing behavior and strengthening one's sense of self are crucial aspects to achieving lasting health and joy. Archway's clinical team can help you determine if our MAT program is right for you.

The ASAM Criteria helps us determine the appropriate level of care for someone seeking addiction treatment. Long experience and evidence-based research help us assure you get the help you need

Principles of Effective Treatment

This tool helps clinicians determine at-risk use among teens. It is best used in consultation with a clinician.

Relapse Prevention & 5 Rules of Recovery

There are four main ideas in relapse prevention. First, relapse is a gradual process with distinct stages. Second, recovery is a process of personal growth with developmental milestones. Third, the main tools of relapse prevention are to develop healthy coping skills in treatment. Fourth, most relapses can be explained in terms of a few basic rules.
