Medication Assisted Treatment Intake Form
Medication-assisted treatment (MAT) is available to support those who struggle to maintain sobriety through counseling and community support alone. FDA-approved medications may be used as an aid to recovery. Coupled with the work of recovery and developing the support of a recovery community, MAT can help. Learning new ways of thinking, changing behavior and strengthening one's sense of self are crucial aspects to achieving lasting health and joy. Relapse Prevention & 5 Rules of Recovery
There are four main ideas in relapse prevention. First, relapse is a gradual process with distinct stages. Second, recovery is a process of personal growth with developmental milestones.
