Medication Assisted Treatment Intake Form
Medication-assisted treatment (MAT) is available to support those who struggle to maintain sobriety through counseling and community support alone. While FDA-approved medications may be used as an aid to recovery, they are not a substitute for the work of recovery. Learning new ways of thinking, changing behavior and strengthening one's sense of self are crucial aspects to achieving lasting health and joy.
