Medication Assisted Treatment Intake Form

Medication-assisted treatment (MAT) is available to support those who struggle to maintain sobriety through counseling and community support alone. FDA-approved medications may be used as an aid to recovery. Coupled with the work of recovery and developing the support of a recovery community, MAT can help. Learning new ways of thinking, changing behavior and strengthening one's sense of self are crucial aspects to achieving lasting health and joy. Archway's clinical team can help you determine if our MAT program is right for you.

Relapse Prevention & 5 Rules of Recovery

There are four main ideas in relapse prevention. First, relapse is a gradual process with distinct stages. Second, recovery is a process of personal growth with developmental milestones. Third, the main tools of relapse prevention are to develop healthy coping skills in treatment. Fourth, most relapses can be explained in terms of a few basic rules.

  1. Create a new life where it is easier to not use

  2. Be completely honest

  3. Ask for help

  4. Practice self-care

  5. Don’t bend the rules.
