HEALTH CARE RECORDS REQUEST

To request health care records from Archway Recovery Services, choose the option that applies to you and follow the steps below.

REQUEST YOUR OWN RECORDS
 

  1. Select “Request My Records” below.
     

  2. Complete the online request form.
     

  3. Upload a valid photo ID.
     

  4. Please allow up to 14 business days for processing.

REQUEST MY RECORDS

REQUEST RECORDS FOR
SOMEONE ELSE

  1. Download and complete the Authorization for Release of Information (ROI) form.
     

  2. Have the patient or their legally authorized representative sign the form.
     

  3. Select “Request Someone Else’s Records” below.
    Complete the online request form.
     

  4. Upload the completed, signed ROI form and any supporting authorization documents, if applicable.
     

  5. Please allow up to 14 business days for processing.
